Six Sigma exams are failed in remarkably repetitive ways. The questions change; the mistakes do not. Candidates walk in with tool knowledge but no roadmap, definitions memorized in isolation, pacing they have never rehearsed, and a proctoring setup they are meeting for the first time. The good news hides in the repetition: a predictable failure is a preventable one. Here are the mistakes that cost the most points, and the habit that neutralizes each.
Mistake 1: Learning the tools without the roadmap
The most common error is studying Six Sigma as a bag of tools — a fishbone here, a control chart there — without the DMAIC structure that gives each tool its place. Exams punish this immediately, because many questions are not about what a tool is but about when it belongs: which phase validates the measurement system, which phase pilots the fix, which deliverable closes Define. A candidate who knows every tool but not the sequence will recognize every term in the question and still pick the wrong answer. The fix is to make the roadmap your filing system: every time you learn anything, file it under its phase, and review by phase rather than by topic.
Mistake 2: Memorizing definitions without their contrasts
Exam writers rarely ask what a term means. They ask which of four similar terms fits a scenario — and that is a different skill. Candidates who studied definitions one at a time discover on exam day that everything sounds plausible. Study confusable terms in pairs, and be able to state the difference between them in one sentence. A few classics repay the effort.
- Common cause versus special cause variation — the routine noise of a process versus a signal that something changed.
- Accuracy versus precision — hitting the true value versus hitting the same value repeatedly.
- Effectiveness versus efficiency — producing the right outcome versus producing it with minimal waste.
- Cycle time versus lead time — the time work is actively processed versus the total time the customer waits.
- Mistake-proofing versus inspection — preventing a defect from occurring versus catching it after it exists.
Mistake 3: Answering from workplace habit instead of the method
Experienced candidates carry a special risk: they answer scenario questions with what their company would do rather than what the methodology prescribes. Your workplace may well skip the charter, improvise the pilot, and call it agility — the exam does not care. When a scenario offers a shortcut that resembles real life and a disciplined step that resembles the textbook, the exam wants the discipline. Answer as the method’s ideal practitioner, not as your office’s survivor.
Mistake 4: Losing points to execution, not knowledge
Pacing is a skill, and like every skill it does not appear on demand. Candidates who never rehearsed under time discover that a 100-question exam has its own physics: a handful of hard questions can quietly absorb a large share of the clock, and the panic that follows damages answers the candidate actually knew. The counter-habits are simple and rehearsable.

Execution errors have a second face: misreading. A share of wrong answers comes from candidates who knew the material and answered a different question than the one asked — a missed not or except, the first plausible option chosen over the best one, a scenario detail ignored that would have eliminated two choices. The habit that prevents it is mechanical: read the stem twice, predict the answer before looking at the options, then eliminate rather than select. It costs seconds and saves points.
Mistake 5: Improvising the proctored setting
A proctored online exam has rules, and candidates who skim them create their own worst moments: a phone left on the desk, an interruption mid-exam, a camera that fails at check-in. None of this is about knowledge, and all of it drains composure at the exact moment composure pays. Do the boring preparation — test your equipment on the machine you will use, choose a private room, clear the desk, have identification ready, and read the rules the day before. The candidates who treat check-in as routine are the ones who made it routine in advance.
Mistake 6: Treating the retake as the plan
Knowing a free retake exists changes behavior, and not always for the better. Some candidates half-prepare, reasoning that the first attempt can serve as reconnaissance. This wastes the attempt and, worse, rehearses failure. The retake is a backstop, and backstops work best when they are not part of the plan. Prepare to pass the first time; then, if the exam surprises you anyway, use the retake the way a practitioner would — study exactly where the first attempt exposed weakness and return within weeks, while the material is warm.
