DPMO — defects per million opportunities — expresses how often a process produces defects, normalized for how many chances each unit offers. The formula: divide total defects by total units times opportunities per unit, then multiply by one million. The normalization is the point. It lets a simple process and a complex one be compared on the same scale.
How it works
An opportunity is a distinct chance for a requirement to be missed on a single unit. A five-field form has five opportunities per form; a wire transfer might have a dozen. Because complex units carry more opportunities, raw defect counts flatter simple processes and punish complex ones. DPMO corrects for that, which is why it underpins the sigma-level scale — six sigma performance corresponds to 3.4 DPMO under the standard convention.
The number is only as honest as the opportunity count behind it. Definitions must be set before measuring begins and held fixed afterward.
DPMO also earns its place in reporting. Percentages blur at high performance — 99.4 percent accurate sounds interchangeable with 99.98 percent, yet the two differ by a factor of thirty in defects delivered. Stating rates per million keeps that difference visible, which is precisely what a team needs once the easy defects are gone and the remaining ones hide in the decimals.
A worked mini-example
An illustrative clinic audits 200 patient intake forms, each with five checked fields — five opportunities per form, 1,000 opportunities in total. The auditors find 12 errors across the sample. DPMO equals 12 divided by 1,000, times one million: 12,000. Whether that number is alarming depends entirely on the field it describes: for a marketing preference, perhaps tolerable; for allergy information, not remotely. DPMO states the rate; the CTQ decides what the rate must be. The clinic can also convert the figure to a sigma level using a standard table, putting intake forms on the same scale as any other process it measures.
- Fix the opportunity definition before collecting data, and write it down
- Resist opportunity inflation — adding opportunities lowers DPMO without improving anything
- Compare DPMO only across processes with comparable opportunity definitions
- Recalculate from scratch after a process change; old opportunity counts may no longer apply
- Treat DPMO as a diagnostic, not a target to be gamed
DPMO is honest exactly as long as the opportunity count is.
